Care@Home is designed to keep you living your best life possible.
Our Care Team will come to your home at any time-of any day to make sure you are getting the care you need.
The Care Team will work with you, your caregivers or loved ones, your Primary Care Physician (PCP) and other doctors to create a Care Plan that best fits your needs and care goals. Goals, like helping you manage your chronic conditions, potentially reduce the number of medications you are taking, trips to the emergency room and hospital stays.
The Care@Home Care Team includes:
Doctor (MD)
Nurse practitioner (NP)
Nurse case manager
Behavioral health specialist
Care coordinator
Social worker
Registered dietitian
Pharmacist
We work with your PCP and other doctors to coordinate the care you need and keep them in the loop about your care at all times.
Our doctors do not replace your PCP or specialists you are currently seeing.
We provide regularly scheduled and urgent care in-home visits, with 24/7 access to care coordination.
We make sure your appointments are scheduled at a time and day that work best for you.
We bring basic services, like lab tests and mobile X-ray, to you.
We work with you and/or your caregiver for the best possible outcomes by always explaining what we’re doing and why. The more you take part in your care the better the results.
